Use this service to register for proxy access to GP online services.

You can choose to do this for an adult or child.

You can use this service if you:

• are registered at the surgery

Practice policy

It is practice policy to automatically turn off proxy access to a child’s full records once that child reaches the age of 11. We generally only grant full online records access to children once they attain the age of 16.

Before you start

We’ll ask you for:

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• photographic identification and proof of address
