Phoenix Health Group offers a personal list system. This means that when you register with the practice you will have a named GP who will be responsible for your care. You will be expected to see the doctor that you are registered with wherever possible. Patients are welcome to attend their own doctor at any of the surgeries they work in.
If your doctor is not available and you cannot wait for their return, then you can see another doctor.
The personal list system helps with the continuity of care and allows the doctor to get to know you and your family. This helps with our philosophy of providing personalised holistic care. If you wish to know the name of your accountable GP please contact the practice.
If you wish to change your doctor within the practice, please fill in a change of doctor request form, available at Reception, this will then be discussed by the partners.
