Salary £13.50 to £14.00 per hour depending on experience

An exciting opportunity has arisen at our established and busy practice for a Care Coordinator to join our team at Phoenix Health Group. Innovative and progressive, with surgeries across Cirencester and Tetbury, we focus on providing excellent personal care and a holistic approach to medical practice.
As a Care Coordinator you will work closely with our GPs, supporting them to provide personalised care to their patient population.
You will help people to manage their needs through answering queries, supporting them to make and manage necessary appointments with community and secondary care. You will ensure appropriate follow up and support with the referral process; and that they have good quality written or verbal information to help them make informed choices about their care.
You will need:
- Experience in a healthcare setting
- Active and empathic listening with excellent communication and questioning skills
- Ability to build trust and rapport
- Ability to use your own judgement, resourcefulness and common sense
- Ability to work without direct supervision and determine own workload priorities
